The Willows at Ramapo Rehabilitation and Nursing Center is looking for a qualified, hard-working and customer service driven Social Worker with long-term care experience to join their team.

Social Worker Job Description

Looking for a full-time Social Worker that is self motivated with the ability to evaluate and treat independently without direct supervision. Must possess the desire to assist residents improve and should have a passion for working with geriatric population. Must be flexible, detail-oriented, have the ability to multi-task and be a team player.

Social Worker Duties Include but are not limited to:

Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.

Assists the Social Services Director in providing services to meet the social and emotional needs of the residents/patients.

Develops a rapport with the residents/patients and presents their concerns to facility management. Charts as assigned.

Ability to relate positively, effectively and appropriately with residents, families, visitors, and other facility staff.

Ensure and provide support and education to residents/family members/significant others to assist in the understanding of placement and facility issues.

Social Worker Requirements/Qualifications:

• Bachelor's Degree and/or Master's Degree in Social Work or Human Services with geriatric experience.
• LMSW (not required)
• Experience with case management, social work, and/or counseling in a healthcare environment.
• Experience in LTC preferred
• Positive attitude and team mentality
• Must be able to read/write/speak English
• Must be 18 years of age

Hours:

Monday – Friday 9am-5pm

Salary: Up to $60,000 a year
