Social Worker Case Manager

We are seeking a skilled and compassionate social worker to join our team as a Case Manager for individuals on the New Choices Waiver. The Case Manager will be responsible for assisting clients in accessing services and resources, coordinating care with other healthcare professionals, and developing care plans to meet the individualized needs of clients.

Location: This position requires going to clients and can be based in Utah County.

Responsibilities:

• Conduct assessments of clients' needs and develop care plans based on those needs
• Coordinate with healthcare professionals, such as nurses and doctors, to ensure that clients receive appropriate medical care
• Help clients access community resources, such as housing and transportation services
• Provide counseling and support to clients and their families
• Monitor clients' progress and adjust care plans as needed
• Document client interactions and maintain accurate records
• Participate in team meetings and case conferences
• Stay up-to-date with changes in policies and regulations related to the New Choices Waiver program
• Collaborate with other social workers, case managers, and healthcare professionals to ensure the highest quality of care for clients

Qualifications:

• Current licensure as a social worker
• Experience in case management and working with individuals with disabilities
• Knowledge of the New Choices Waiver program and related policies and regulations
• Ability to work collaboratively with other healthcare professionals
• Excellent communication and interpersonal skills
• Strong organizational and time management skills
• Ability to work independently and make decisions in a fast-paced environment
• Compassionate and empathetic approach to working with clients and their families
