Overview

Job Title: Social Worker – MSW / LSW
Location: Methodist Hospital, Indianapolis, IN
Shift: Full-Time Monday 9:30 AM – 6:00 PM; Tuesday – Friday 8:00 AM – 4:30 PM
(No weekends or on-call responsibilities)

Position Overview:
Methodist Hospital in Indianapolis is seeking a dedicated Full-Time Social Worker to join our outpatient adult HIV treatment and prevention team. This position offers a collaborative, multidisciplinary clinical environment where you will work closely with a large social work team and be an essential part of the medical care team. You will manage a caseload of individuals living with HIV, providing ongoing psychosocial assessments, resource linkage, and assistance with grant-specific program applications. Experience in HIV case management is highly preferred.
You will meet face-to-face with patients and families, collaborating with healthcare providers to facilitate safe, timely care and resource provision. You will also serve as a resource, role model, and mentor within the team, supporting patients’ mental health, social needs, and treatment adherence.

Key Responsibilities:
Conduct face-to-face assessments of patients and families to evaluate psychosocial needs, barriers to care, and support systems.
Provide ongoing case management, referrals, and assistance with applications for HIV-related grants and programs.
Collaborate effectively with the multidisciplinary team to ensure comprehensive, patient-centered care.
Assist patients in navigating emotional, social, and financial impacts related to HIV and other health conditions.
Access and mobilize community and family resources to meet individual needs.
Provide mental health assessments and therapeutic interventions to support complex discharge planning.
Serve as the primary point of contact for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, or domestic violence; coordinate with protective agencies as needed.
Support crisis intervention efforts, utilizing de-escalation techniques to prevent violence and manage acute crises.
Conduct interviews and assessments of victims, perpetrators, and significant others, ensuring safety and legal compliance.
Develop and maintain strong relationships with hospital staff, law enforcement, APS/DCS, community agencies, and other stakeholders.
Respond with or in lieu of the Department of Public Safety to de-escalate violence when required.

Qualifications & Requirements:
Education: Master’s Degree in Social Work (MSW) required.
Licensure: Indiana licensure or license eligibility as an LSW or LCSW.
Experience: HIV case management experience highly preferred.
Skills:
Strong psychosocial assessment, referral, and documentation skills.
Ability to work collaboratively within a multidisciplinary team.
Knowledge of mental health, substance use disorders, and health-related social needs.
Experience in crisis intervention and de-escalation techniques.
Ability to adapt services for diverse populations across geographic, cultural, and socioeconomic backgrounds.
Proficiency in electronic medical record documentation.
