SSVF Case Manager

Abilities Required:

Must be proficient with computers
Must be able to communicate clearly and professionally both in the spoken and written word
Strong ability to work as a team as independently
Must possess organizational, planning, information gathering, information monitoring, problem analysis and problem-solving skills
Must possess judgment and decision-making ability, initiative, confidentiality, and attention to detail and accuracy
Must have Associates or Bachelors Degree
Valid NYS Drivers License

Responsibilities:

Complete pre-screens on all Veterans that seek assistance.
Conduct community outreach
Work with other agencies to ensure Veterans in need are being reached.
Compile data for monthly reports
Complete and file all applicable reports provide direct client services
Perform eligibility assessments to determine that Veterans are eligible for SSVF Assistance determined by the CNYVOC, that the Veteran is homeless or at imminent risk.
Provide holistic and comprehensive case management services to all clients, with a strong focus on SSVF tenants: including intake assessment, benefit assessment, goal setting, long –term case management, tenant education, advocacy, and referrals.
Assist the Housing Director with the maintenance of the housing binders, tenant leases, and other paperwork
Assist the Housing Director in monthly housing inspections
Identify opportunities for collaborative ventures with local apartment complexes, rental agencies, and owners of rental houses to begin the process of finding local affordable housing for Veterans.
Ensures all applicable regulations and guidelines are followed before requesting approval for assistance.
Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
Help all Veterans and their families that are in need.
Attend community-based meetings when needed.
Work closely with the VA, DOL and any other government agencies and community agencies as needed.
Complete SOAR applications for eligible clients

Pay: $35,000.00 - $40,000.00 per year
