Temecula Recovery Center, a Generations Healthcare facility, is looking for a Substance Use Disorder (SUD) Case Manager to join our team.
Position Overview:
The Substance Use Disorder (SUD) Case Manager is responsible for the overall coordination of care and supporting clients/clients through their recovery journey. The SUD Case Manager collaborates with clients, the treatment team, and external providers to coordinate discharge and aftercare plans, ensuring a smooth transition to the next level of care. They regularly meet with clients to assess needs, identify suitable referrals, and secure aftercare appointments. The Substance Use Disorder (SUD) Case Manager will also be cross-trained and scheduled for Behavioral Health Technician (BHT) shifts.

Key Responsibilities:
  • Work cohesively with the Clinical Outreach Team to coordinate transitioning levels of care.
  • Participate actively in interdisciplinary treatment team meetings, providing input on client progress and care needs.
  • Develops, implements, manages, and continually evaluates individualized client care plans and discharge plans.
  • Maintain regular communication with families, referral sources, probation/parole officers, and other stakeholders, as appropriate and with proper consent.
  • Assess each client’s social, employment, and medical needs, and facilitate necessary appointments and paperwork whenever possible.
  • Assist clients in accessing community resources such as housing, employment, education, transportation, and financial assistance.
  • Advocate for client’s needs within the healthcare, legal, housing and employment systems.
  • Completes all documentation required within the outlined timeframes, prior to client’s discharge from the facility.
  • Regularly meets with clients to assess progress, at a minimum of once weekly, and adjust care plans, as needed.
  • Facilitate Case Management groups, and additional groups as needed.
  • Provide crisis management and intervention as needed.
  • Maintain accurate, confidential, and up-to-date records, and completes charting in the Electronic Medical Record (EMR).
  • Monitor compliance with court-ordered, probation, or other external requirements when applicable.
  • Provide psychoeducation to clients on recovery-related topics (e.g., relapse prevention, life skills, coping strategies).
  • Support client engagement in 12-step, SMART Recovery, or other community-based recovery support groups.
  • Coordinate with medical staff regarding medication-assisted treatment (MAT) follow-up and adherence.
  • Participate in ongoing training and professional development to remain current with best practices in SUD treatment and case management.
  • Provide mentorship and support to Behavioral Health Technicians during cross-trained shifts, promoting consistent client care standards.
  • Perform additional duties, as assigned by the Clinical Director or Administrator.

Educational and Experience Requirements:

  • High school diploma or equivalent required
  • Minimum of one year of case management or equivalent role experience in a substance use disorder treatment program or other mental health setting required.
  • Registered through California Consortium of Addiction Programs and Professionals (CCAPP), Addiction Counselor Certification Board of California (ACCBC), California Association for Alcohol/Drug Educators (CAADE), or California Association of DUI Treatment Programs (CADTP) preferred.
  • Registered Drug and Alcohol Technician (RADT), Certified Alcohol & Drug Counselor Level I (CADC-I) or similar certification preferred.
  • Certification in approved de-escalation and restraint techniques (e.g., CPI, Mandt, or equivalent), or ability to obtain within 90 days of hire.
  • Valid CA Driver’s License with ability to pass a Motor Vehicle Record (MVR) check.
  • Current Adult CPR certification required.
  • Current Adult First Aid certification required.
  • This position requires the successful completion of a background check, pre-employment physical, and drug screening as part of the hiring process.

Knowledge, Skills, and Abilities:

  • Knowledge of relevant regulations, policies, and ethical guidelines related to substance use treatment preferred.
  • Knowledge of basic principles, practices and techniques of mental health and drug and alcohol intervention.
  • Knowledge of Joint Commission Standards preferred.
  • Knowledge of Title 9 Licensing Regulations and DHCS AOD Certification Standards preferred.
  • Knowledge of state & federal statutes regarding client confidentiality laws preferred.
  • Proficient in Microsoft Office 365 (Outlook, Word, Excel, PowerPoint, Teams, SharePoint) or similar software programs.
  • Comfortable using email, calendars, and task management tools in Outlook and Teams.
  • Strong keyboarding and general computer navigation skills.
  • Experience maintaining digital files, folders, and permissions in a secure manner.
  • Knowledge of currently utilized technology and EMR software (i.e., PCC, Kipu) preferred.
  • Excellent verbal & written communication skills.
  • Ability to work as a team member in a collaborative, fast-paced work environment.
  • Excellent communication, organizational and interpersonal skills.
  • Ability to work under stressful conditions and be flexible in relation to department needs.
  • Cultural sensitivity and desire to work with an ethnically diverse adult population.
  • Must be able to read, write, speak, and understand the English language.
  • Superior interpersonal, human relations, and behavior management skills.
