SUD Housing Case Manager

Exodus PCS is a fast-growing and highly regarded provider of the SUD Moving Home Minnesota programs. This role focuses on housing identification, pre- and post-discharge transition planning, service coordination, and long-term housing stability, utilizing person-centered, recovery-oriented, and trauma-informed practices.

We offer paid training to help Case Managers develop the skills needed to succeed and navigate relocation and community programs. The Case Manager works closely with lead agencies, treatment providers, housing partners, and community supports to ensure safe, successful transitions from qualified institutions into integrated community settings, consistent with MHM program requirements.

Key Responsibilities:

• Assist residents locating and securing appropriate housing options in the community.
• Coordinate and collaborate with service providers, community resources, and families to develop individualized transition plans.
• Assist clients in identifying, securing, and transitioning into qualified community residences, including leased apartments, family or small residential settings.
• Support clients through discharge planning, housing coordination, and case management.
• Consistent communication with support team and completion of case documentation
• Maintain accurate client records using Microsoft 365 and Microsoft Teams.
• Provide assistance during discharge days, including light lifting (up to 25 lbs.).

Qualifications:

• Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, Nursing, or a related field.
• At least two years of experience in human services, social services, or healthcare.
• Knowledge of community resources, waiver programs, and county applications.
• Experience with medical assistance, discharge planning, and housing coordination.
• Clearance to work in human services through a DHS background check.
• Reliable vehicle with valid driver’s license and insurance.
• Proficiency with Microsoft 365 and Teams.

Location:
Exodus PCS is based in Burnsville, MN. The first few weeks of training will take place in the main office. After training, work is hybrid, with some being completed remotely from home and some in the community on client visits.
