Overview
We are seeking a dedicated and skilled T-LPN / LPN to join our healthcare team. This role involves providing high-quality nursing care across various clinical settings, including acute, long-term, and community-based care environments. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and medical documentation, with experience working with diverse patient populations such as seniors, individuals with developmental disabilities, and those requiring specialized care like Alzheimer’s or dementia care. Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks is preferred. The T-LPN / LPN will play a vital role in delivering compassionate patient care while adhering to HIPAA and infection control standards.

Responsibilities

  • Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history documentation
  • Administer medications and injections accurately following physician orders and safety protocols
  • Perform IV infusions, catheterizations, and sterile procedures with aseptic technique
  • Manage airway support including ventilator care and airway management techniques
  • Assist with diagnostic evaluations such as laboratory specimen collection, processing, and sonography procedures
  • Provide specialized care for patients with conditions such as Alzheimer’s disease, dementia, or behavioral health needs
  • Document all patient interactions thoroughly within EMR/EHR systems like Epic or Cerner
  • Collaborate with multidisciplinary teams for discharge planning, case management, and post-acute care coordination
  • Support patients in rehabilitation settings including ICU, PICU, NICU, or trauma centers
  • Educate patients and families on health coaching topics including nutrition, medication management, and chronic disease management
  • Ensure compliance with infection control policies and maintain sterile environments during procedures

Experience

  • Proven experience working in diverse healthcare settings such as hospitals (Level I Trauma Center preferred), emergency rooms, ICU/PICU/NICU units, outpatient clinics, or home & community care environments
  • Strong background in critical care areas including ICU experience or emergency medicine is highly desirable
  • Familiarity with ICD-9/10 coding and CPT coding for accurate medical documentation
  • Experience administering injections, phlebotomy, venipuncture, catheter care, and tube feeding procedures
  • Knowledge of physiology and anatomy relevant to patient assessment and diagnostic evaluation
  • Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is preferred
  • Ability to work with special populations including pediatrics (including neonatal ventilators), geriatrics (memory & Alzheimer’s care), behavioral health patients, or individuals with disabilities
  • Demonstrated understanding of HIPAA regulations and infection control standards to ensure patient privacy and safety
