Targeted Case Manager (Telehealth)

A Part-Time Targeted Case Manager at Kentucky Counseling Center will work one-on-one with clients who have Medicaid to set and obtain personal goals, financial assistance, and consistent mental health care. This position is telehealth.

The Part-Time expectation is to complete 13 billings per month, in accordance with Medicaid regulations and requirements and Kentucky Counseling Center company standards of quality care and coordination.

Targeted Case Managers will work on a team of service providers that find resources, assist in action, and provide needed education and information to their clients in order to meet their needs and goals.

Must have experience with targeted case management in behavioral, mental health, or co-occuring populations. 12hr core training + SED, SUD, and CCPCH training must be completed within 90 days.

Applicants with natural communication skills, genuine interest in helping others, goal-oriented mindsets and strategies, and independent organization and time management skills are encouraged to apply. BA or BS in a Human Service field is required, a MA or MS in a Human Service or Social Work field is ideal.
