TEMP Case Manager - Rockland Treatment Alternatives for Safer Communities (TASC)

EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated temporary part time Case Manager to join the Rockland County Treatment Alternatives for Safer Communities (TASC) Program until 12/31/2026.

  • Wage: $30 per hour

  • Schedule: up to 19 hours per week

  • Location: 1 S. Main Street Suite 500 in New City, NY

What drives the core purpose of this job:

The Case Manager provides screening, treatment planning, case management, behavioral interventions and court reporting of eligible substance abusing and/or mentally ill (co-occurring) clients.

What are the key responsibilities:

  • Act as liaison to the courts and correctional facilities. Provide screening services and conduct assessments to determine behavioral health needs.
  • Complete referrals to link clients to the appropriate program in the community to address their substance abuse, mental health and/or behavioral health needs.
  • Make monthly site visits to all treatment agencies for assigned clients.
  • Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
  • Maintain a caseload of clients and monitor their participation in the treatment program through case management appointments, reviewing progress reports, and conferencing cases with treatment personnel.
  • Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.
  • Investigate and recommend supportive services and behavioral interventions for clients.
  • Make timely visits to treatment and correctional facilities and attend meetings as needed.

What key knowledge, skills, and abilities are required to excel in this role:

  • Minimum of an Associate degree with 1 year experience working in the behavioral health field required or Bachelor’s degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology and/or a significant combination of education and experience in either mental health, substance abuse or criminal justice.

  • Good communication skills, both oral and written. Ability to work cooperatively with treatment and criminal justice systems.

  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
