Transplant Coordinator
Key Responsibilities:
- Patient Coordination:
- Manage the care of transplant candidates, recipients, and donors throughout the continuum.
- Conduct patient assessments, education, and counseling.
- Coordinate patient testing, diagnostics, and consults.
- Clinical Duties:
- Monitor and document patient status, progress, and complications.
- Participate in organ offer evaluations and donor-recipient matching.
- Coordinate transplant surgery logistics with surgical and medical teams.
- Administrative Tasks:
- Maintain accurate patient records in accordance with hospital, state, and national guidelines (NOTTO).
- Assist with data collection and reporting to regulatory bodies.
- Participate in quality assurance and performance improvement programs.
- Communication and Collaboration:
- Liaise with physicians, nurses, social workers, and other healthcare staff.
- Serve as a point of contact for patients and families.
Medical Social Worker
Key Responsibilities:
- Psychosocial Assessment:
- Evaluate patient and family psychosocial functioning, support systems, coping skills, and financial readiness for transplant.
- Provide recommendations to the transplant team regarding suitability and potential barriers.
- Counseling & Support:
- Offer individual and family counseling related to stress, anxiety, grief, and adjustment.
- Provide crisis intervention as needed.
- Resource Coordination:
- Help patients access financial aid, housing, transportation, insurance, and other community resources.
- Collaborate with financial counselors and patient navigators.
- Patient Education:
- Educate patients and families on the psychosocial aspects of transplantation, including lifestyle changes, compliance, and support networks.
- Interdisciplinary Collaboration:
- Participate in multidisciplinary meetings, case conferences, and discharge planning.
- Advocate for patient needs within the healthcare system.
