Responsibilities

• Interpret medication orders, accurately transcribe to and maintain computerized patient medication profiles
• Accurately dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition products
• Comply with all applicable federal, state, and local laws; manage controlled substances and maintain required records
• Monitor drug therapy regimens for interactions, allergies, dosing appropriateness, and contraindications
• Perform pharmacokinetic dosing according to hospital protocol; review antibiotic therapy and interpret culture and sensitivity data
• Provide clinical consultation to practitioners, offer cost-effective therapeutic alternatives, provide drug information to patients and families, and document all clinical activities and interventions
• Participate in quality improvement, medication use evaluation, and education initiatives; support code response activities as a team member

Requirements

• Active Colorado Pharmacist License in good standing (CO PHA)
• BS Pharmacy degree from an ACPE-accredited program
• Current BLS and ACLS certifications (American Heart Association)
• Flu vaccine on file with no exemptions required (seasonal requirement Oct 1 through Apr 1)
• Epic EMR experience is required; Meditech training is a plus
• Ability to work in a hospital setting and participate in rotating day/evening shifts, including weekends
• Radius rule: permanent address at least 75 miles away from the facility location
• Submissions should include: resume, proof of Epic experience, skill sheet, proof of flu vaccination, BLS card, ACLS card, and CO PHA license
