Description

American Traveler is seeking a licensed Physical Therapist for a 13-week home health travel assignment in the Fremont, CA area, requiring a minimum of 1 year of experience and a current CA PT license.

Details

  • Home health setting serving patients across a defined county territory including Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont
  • Day shift schedule
  • Weekend coverage may be required to maintain weekly visit expectations
  • Occasional travel outside of the primary service area may be required
  • Caseload includes routine visits, evaluations, discharges, re-evaluations, and OASIS assessments (SoC, ReCert, RoC)
  • Must be willing to see COVID-positive and COVID PUI patients

Requirements

  • Current California PT license required (license required if applicable)
  • Minimum 1 year of physical therapy experience
  • Current BLS certification required
  • Valid driver's license, current auto insurance, and current vehicle registration required
  • Annual TB test required
  • MMR, Varicella, HepB, and TDap (within 10 years) immunizations required
  • Annual flu vaccination required
  • Drug screen and physical required

Additional Information

  • Responsibilities include patient evaluations, routine visits, OASIS assessments, discharge planning, documentation, and care coordination via phone with patients and physicians
  • All requested time off (RTO) must be disclosed at the time of submission; RTO requested after acceptance will not be approved
  • Former employees of the facility are not eligible for this travel assignment
  • A smartphone and medical devices (stethoscope, pulse ox, gait belt, blood pressure cuff) will be provided for field use
