American Traveler is seeking an experienced RN Hospice Case Manager for a community-based home hospice position in Wisconsin, requiring a minimum of 2 years of experience and an active WI or compact RN license.

Community-based home hospice setting serving patients in private homes, assisted living facilities (ALFs), SNFs, group homes, independent living communities, and two hospitals for general inpatient (GIP) care

Patient population consists of end-stage diagnoses requiring hospice-level care
-
RN caseload of 14–16 patients per RN, with a team of 7 RNCMs and 6 CNAs

Uses Epic EMR and Rover application; IV pumps, medication dispensing systems, and chair/bed alarms are utilized

Carries home health bag and trunk supplies for community patient visits

Monday through Friday, 8:00 AM to approximately 4:30 PM (1st shift, 40 hours/week)

No weekend rotation, no on-call, and no holiday coverage required

"Floating" consists of traveling within the community to patient homes

Active WI or compact RN license required

Current BLS certification required

Valid driver's license and active auto insurance required

Minimum 2 years of experience in hospice or relevant specialty area, with most recent experience within the last year

Experienced travelers preferred; first-time travelers are less likely to be considered

Epic EMR experience preferred

One professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year required for consideration

Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

Serves as primary RN Case Manager for a caseload of end-stage hospice patients across multiple community settings

Unit culture emphasizes teamwork and collaborative team support

1-week unit orientation provided

Blue or black scrubs required
