Description

American Traveler is seeking an experienced RN Hospice Case Manager for a community-based home hospice position in Wisconsin, requiring a minimum of 2 years of experience and an active WI or compact RN license.

Details

  • Community-based home hospice setting serving patients in private homes, assisted living facilities (ALFs), SNFs, group homes, independent living communities, and two hospitals for general inpatient (GIP) care
  • Patient population consists of end-stage diagnoses requiring hospice-level care
  • RN caseload of 14–16 patients per RN, with a team of 7 RNCMs and 6 CNAs
  • Uses Epic EMR and Rover application; IV pumps, medication dispensing systems, and chair/bed alarms are utilized
  • Carries home health bag and trunk supplies for community patient visits
  • Monday through Friday, 8:00 AM to approximately 4:30 PM (1st shift, 40 hours/week)
  • No weekend rotation, no on-call, and no holiday coverage required
  • "Floating" consists of traveling within the community to patient homes

Requirements

  • Active WI or compact RN license required
  • Current BLS certification required
  • Valid driver's license and active auto insurance required
  • Minimum 2 years of experience in hospice or relevant specialty area, with most recent experience within the last year
  • Experienced travelers preferred; first-time travelers are less likely to be considered
  • Epic EMR experience preferred
  • One professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year required for consideration
  • Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

  • 1-week unit orientation provided

  • Local candidates are accepted at the same rate; no radius restrictions apply
  • Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible for this travel position
