Responsibilities

• Transcribe detailed clinical notes during patient visits, capturing symptoms, diagnoses, treatment plans, and provider instructions with accuracy and efficiency
• Assist healthcare providers by managing electronic health record (EHR) and electronic medical record (EMR) systems to input and organize patient data seamlessly
• Ensure all medical documentation complies with legal, regulatory, and organizational standards for confidentiality and accuracy
• Support providers by updating patient histories, medication lists, lab results, and imaging reports in real-time during appointments
• Review and verify medical records for completeness and correctness after each encounter to maintain high-quality documentation standards
• Maintain a thorough understanding of anatomy, physiology, and medical terminology to accurately interpret provider dictations and notes
• Collaborate with healthcare teams to improve documentation workflows and optimize the use of EMR/EHR systems for better patient outcomes

Qualifications

• Proven experience in medical office settings or as a transcriptionist with familiarity in medical documentation practices
• Strong knowledge of medical terminology, anatomy, physiology.
• Proficiency with EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems for efficient data entry and management
• Excellent typing skills with high accuracy and speed to keep pace with live dictations during patient visits
• Ability to work independently in a remote setting while maintaining excellent communication with healthcare providers
• Knowledge of medical records management regulations and confidentiality standards
