Wellness/Cannabis Provider

Qualified Medical Provider (NP/PA) – Medical Cannabis & Wellness

We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Empathetix Wellness LLC. This position involves evaluating and treating patients for medical cannabis and wellness services, including weight loss, hormone replacement, and ketamine treatments.

This position is primarily based in Ogden, with a consistent weekly schedule, and includes occasional coverage at our Millcreek and South Jordan clinics. Candidates must be comfortable traveling between locations when scheduled.

1. Duties & Responsibilities (CANNABIS)

• Evaluate patients for medical cannabis eligibility and provide appropriate recommendations.
• Educate patients on the safe and effective use of medical cannabis.
• Maintain accurate and compliant patient records.

2. Duties & Responsibilities (WELLNESS)

• Provide weight loss treatments, including injections.
• Oversee ketamine therapy for mental health and chronic pain management.
• Administer testosterone therapy and hormone replacement therapy (HRT).
• Perform lab draws and review results with patients.

3. Required Licenses & Certifications for Medical Cannabis Providers in Utah

• Must be a Physician Assistant (PA), or Nurse Practitioner (NP).
• Must hold an active and unrestricted Utah medical license.
• Providers must complete the Utah Department of Health & Human Services (DHHS) QMP training.
• Apply for QMP certification through the Utah Medical Cannabis Program.
• Certification allows the provider to recommend medical cannabis.

4. DEA & Controlled Substance License (Utah DOPL)

• DEA license must be registered under Empathetix Wellness LLC for prescribing.
• Must obtain a Utah Controlled Substance License through DOPL.

Job Type: Part-time

Pay: $60.00 per hour
