1115 Waiver Enhanced Navigator - Fulton and Montgomery Counties

As the 1115 Waiver Enhanced Navigator for Catholic Charities of Fulton and Montgomery Counties, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver

using the approved ACH screening. Ensure all demographic information is accurately collected and member consent is properly documented.

  • Ensure timely and accurate data entry of all services, interactions, case notes, and outcomes in the Unite Us platform in accordance with 1115 Waiver requirements
  • Provide education to the community, referral partners, and clients regarding eligibility criteria and available services under the waiver.
  • Participate in all required trainings associated with the 1115 Waiver, including but not limited to: Unite Us platform, trauma‐informed care, culturally and linguistically appropriate services, HIPAA, and compliance.
  • Collaborate with other Case Managers and Catholic Charities team members to ensure coordinated, client‐centered service delivery

1115 Waiver Enhanced Navigator Minimum Salary: $22.00 / Hour

1115 Waiver Enhanced Navigator Maximum Salary: $22.00 / Hour

1115 Waiver Enhanced Navigator Schedule: Monday – Friday 8:00AM – 4:00PM (35 hours per week)

Qualifications:

  • Associates Degree in Human Services or related field
  • 1-3 years of experience in mental health, substance use, or human services
