Home Care Liaison
Collier County, FL
Extended Family is a premier, fast-growing private duty (non-skilled) home health care agency. The agency is based out of Sarasota with a current service area that spans from North Sarasota to Naples. This role will have Collier County as its territory and will have an UNCAPPED monthly commission structure based on recurring client revenue and a quarterly bonus plan based on starts of care. The agency is only accepting applicants with outside sales experience in senior home health. The Home Care Liaison’s key initiative is to accelerate the agency’s growth by building relationships with referral sources in the agency’s service area. This position will attend sales networking events in the area to represent the agency’s brand and will build close relationships with key referral sources to maximize new client leads and build a business pipeline. The position will report to the agency’s Chief Executive Officer and will also work closely with the Chief Financial Officer to stay aligned regarding the agency’s staffing levels, client needs, weekly KPI targets, and business development strategy. A key aspect of this position will be to implement referral source pipelines to build the agency’s brand and drive revenue to attain continued business growth.
Detailed Job Responsibilities:
· Facilitate growth of client census through creation and fostering of relationships with key referral sources.
· Attend industry networking events in the area to represent the agency and make connections.
· Prospect Directors of Health/Wellness and Case Managers of target facilities in the area to build close relationships.
· Document referral sources, prospects, and activities on a weekly basis in the agency’s Customer Relationship Management (CRM) system.
· Conduct in-home client consultations upon request in a timely manner.
· Determine program eligibility by verifying needed services, ability to pay, and educating clients about the long-term care insurance reimbursement process if they are a policyholder.
· Educate prospects about the agency’s services and pricing, along with what sets the agency apart from its competitors.
· Document client needs obtained during consultation and provide the information to the Nurse Supervisors and Care Coordinators to facilitate initial assessment and scheduling.
· Review completed client service agreements for accuracy and forward to Administrative team for processing.
· Follow up with clients regarding their service experience to maintain relationships and ensure that each client feels like their needs are heard and met.
· Develop and implement strategies to enhance the agency’s market position while fostering its mission and culture.
· Brief the Executive team on the status of prospects, new clients, and existing clients during weekly office meetings.
Compensation & Benefits:
· $120,000 Base Salary.
· $150 Weekly Auto Allowance.
· UNCAPPED monthly commissions based billed revenue from starts of care (2% - 4% of recurring revenue for the client’s entire length of service).
· Quarterly bonuses based on starts of care – ($3,000 - $7,500 potential each quarter).
· Eligible for benefits (Health, Dental, Vision) 30 days after hire.
· Eligible to participate in 401(k) Plan 30 days after hire.
· Paid time off - 15 days annually.
· Hybrid schedule (weekly in-office meetings, field work, and documenting CRM & conducting follow ups remotely).

Qualifications:
Certifications and Licensures: State of Florida Driver's License (Required)
Education: Bachelors Degree in Business Marketing (Preferred)
Experience: Minimum of two years of sales experience in home health care (Required)
Job Type & Schedule: Full-time, Monday to Friday 9:00am – 6:00pm
Work Location: On the road in Collier County.
Ability to commute/relocate: Able to reliably commute within agency’s service area of Collier County, FL (Required).
