Case Manager

The Case Manager provides community-based support and coordination of services for program participants. This role is responsible for assessing participant needs, connecting individuals with community resources, and supporting the implementation of individualized recovery or service plans. The Case Manager works collaboratively with multidisciplinary teams to promote recovery, stability, and independence for participants.

Key Responsibilities

  • Coordinate and implement individualized service and recovery plans.

  • Provide community-based case management services and support participants in achieving personal goals.

  • Assist participants with accessing resources such as housing, medical care, employment, education, and behavioral health services.

  • Advocate on behalf of participants to ensure access to necessary supports and services.

  • Transport participants to appointments and community resources when required.

  • Collaborate with clinical staff, community providers, and other stakeholders.

  • Maintain accurate and timely documentation in accordance with program and regulatory requirements.

  • Identify service gaps and communicate recommendations for improving participant services.

  • Attend team meetings, supervision, and required trainings.

Qualifications

Education:
Bachelor’s Degree in Human Services or a related field preferred, or equivalent experience.

Experience:

  • Two (2) years of experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disability services required.

  • One (1) year of case management experience preferred.

Additional Requirements:

  • Valid driver’s license for a minimum of three (3) years with an acceptable driving record.

  • Ability to work in community settings and transport participants as needed.

  • Intermediate computer and documentation skills.

  • Must obtain and maintain required background clearances, including Child Abuse Clearance, Criminal Background Check, and FBI Clearance (if applicable).

Work Environment

This position includes work in both office and community settings within the program service area and may require travel throughout the community.

  • Ability to drive and travel within the community

  • Ability to lift up to 50 pounds

  • Ability to sit, stand, bend, and climb stairs as needed
