Case Manager / Case Worker (Mental Health)

Job Overview
We are seeking a passionate and dedicated CBRS (Community-Based Rehabilitation Services) Specialist to empower individuals with disabilities and support their journey toward greater independence and community integration. In this vital role, you will work directly with clients in community or home-based settings to facilitate skill development, and independent living related to mental health recovery. Responsibilities

• Work one-on-one with clients to build skills related to daily functioning and independence
• Support development of coping strategies and emotional regulation skills
• Help clients improve social skills, organization, and life management skills
• Assist clients with community integration and access to resources
• Document services and client progress accurately and in a timely manner
• Collaborate with Therapists, Case Managers, and other providers involved in client care
• Maintain professional boundaries and adhere to confidentiality standards

Requirements

• Bachelor's degree in Social Work, Psychology, Human Services, or related field (or equivalent experience)
• Strong interpersonal and communication skills
• Ability to build rapport with clients and support their personal growth
• Valid driver’s license and reliable transportation for community-based work.
• Ability to maintain accurate documentation
• Prefer Experience working in behavioral health or social services
• Prefer experience supporting individuals with mental health challenges
• Ability to work independently as well as collaboratively within a multidisciplinary team.
