Case Manager-Hospice - Registered Nurse - $2,488 per week

Experience

• 12 months of role experience is required with some in the last 12 months.
• Must have at least 12 months of Case Management experience.
• Experience with Homecare Homebase (HCHB) is preferred from any number of months.

• Candidates must have a California license (required for submission).
• Local & travel allowed.

Case Manager-Hospice

Required Skills/Experience: 1 Year of Hospice Nursing Experience Required.

Shift & Scheduling: 8:30am-5pm (Schedule can be flexible based on patient’s availability.) Weekends: One weekend per month. Holiday coverage required.

Floating Requirements: Territory Details: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy and neighboring cities as needed.

7 days maximum time off

1 reference from last 12 months (Manager/Supervisor reference type) - required for submission
