Children's Supports Coordinator

Supports Coordinator, start today! Team Wellness Center is looking for Bachelor- degreed professionals to assist adults with autism achieve their personal goals, so that they may lead a wholesome and fulfilled life. Work with a team of psychiatrists, clinical therapists, primary care physicians, and nurses, to provide the full scope of psychiatric and social work necessary for the person served. You will serve as the point person on the treatment team that assures the member is linked to all services needed and to help coordinate efforts so that they’re safe, housed, fed, and connected to the community.

Requirements:

• Bachelor's Degree in social work or related field
• Experience with adults with Autism required
