- Nutritional Assessment: Conduct comprehensive evaluations of patients' health needs, including analyzing medical records, lab results, dietary intake, and lifestyle factors.
- Care Plan Development: Create and implement individualized, evidence-based meal and nutrition plans that consider medical conditions, allergies, preferences, and cultural backgrounds.
- Monitoring and Evaluation: Track patient progress and outcomes, adjusting nutrition plans as necessary to ensure optimal health outcomes and recovery.
- Patient Education and Counseling: Educate and counsel patients and their families on proper nutrition, healthy eating habits, and the relationship between diet and disease management (e.g., diabetes, heart disease, kidney failure).
- Interdisciplinary Collaboration: Work closely with physicians, nurses, social workers, and other healthcare professionals to integrate nutritional care into the overall patient treatment plan and discharge planning.
- Documentation: Maintain accurate and detailed patient records, documenting all assessments, interventions, and progress in electronic health record (EHR) systems.
- Food Service Liaison: Advise hospital or facility catering departments to ensure meals meet specific patient dietary requirements and standards
