Clinical Pharmacist will ensure the rational and effective use of medication, dispenses medications and prescriptions in accordance with accepted standards and established Pharmacy procedures, patient counseling, monitor patient drug therapies, and provide drug information. To implement national & international standards
KEY RESPONSIBILITIES:

Prepares and dispenses medications.

Contacts prescribers regarding unclear or problem medication orders.

Reconciles patient profiles and medication administration records.
-
Transcribes orders into computerized patient medication profiles.
-
Review drug therapy regimens for contraindications, therapeutic duplications, drug- drug interactions, drug–food interactions, drug–disease interactions, drug–laboratory interactions, allergies, and appropriateness of drug and dose.

Promotes the appropriate, safe, and effective use of antibiotic by reviewing individual patient's medical records and medication regimens for adverse drug reactions, drug interactions, appropriate indications, contraindications, drug duplications, pharmacokinetically sound dosing, and optimal drug selection

Audit and review the use of prescribing and administration antibiotic guidelines, including the surgical antibiotic prophylaxis

Communicate with health care providers to ensure the most appropriate Selection, administration and monitoring of antibiotic for individual patients.

Recommends evidence-based medication therapy regimens and monitoring plans

Detects, and reports suspected adverse drug reactions.

Report medication errors

Review records of patients to determine the appropriateness of medication therapy.

Documents all clinical activities and interventions accurately and completely.

Advise on the correct administration of drugs.

Collaborate with healthcare professionals to ensure optimal patient care

Answers the telephone, identifying self and department. Direct calls to appropriate personal.

Provides drug information and assists in instructing nursing and medical.

Performs inspection of nursing units, discusses service problems.

Develops and coordinates the Quality Improvement Program within his/her area of responsibilities.

Attends orientation, education, and training programs provided for the pharmacy staff.

Participates in the development and presentation of orientation, education, and training programs provided to the pharmacy, medical, nursing, and other staffs

Maintains a clean working environment.

Fulfill the requirement of 24 hours pharmacy service within the Pharmacy schedule.

Serves as a role model and represents the department in a professional manner.

Adhere to the Dress and Appearance Code.

Follows all hospital related policies and procedures.

Performs other applicable tasks assigned within the level of his/her competence as required by Pharmacy Manager.
