Job Title: Community Outreach Coordinator (Part-Time)

Position Overview
Lock Hearts Home Care Solutions is seeking a motivated and relationship-focused Community Outreach Coordinator to help expand awareness of our services and strengthen referral partnerships throughout the community.
This role is responsible for building professional relationships with referral sources, promoting our residential and support services, and increasing access to services for individuals and families in need.
The ideal candidate is a strong communicator who enjoys networking, developing partnerships, and representing an organization within the community.
Key Responsibilities
● Develop and maintain relationships with community referral sources including social workers, case managers, healthcare providers, schools, and local organizations.
● Conduct regular outreach meetings with potential referral partners and community stakeholders.
● Represent the organization at community events, resource fairs, and networking opportunities.
● Coordinate presentations or informational sessions about agency services for community partners and families.
● Track outreach activities, referral sources, and community contacts.
● Support marketing initiatives such as distributing promotional materials and assisting with outreach campaigns.
● Collaborate with leadership to identify opportunities for program growth and community partnerships.
● Monitor referral trends and provide feedback on community needs and service opportunities.

Qualifications
● 3–5 years of experience in community outreach, marketing, business development, healthcare outreach, or nonprofit engagement
● Experience in healthcare, human services, or developmental disability services is highly preferred
● Valid driver’s license and reliable transportation for local travel
Education
Bachelor’s degree preferred in one of the following areas:
● Communications
● Marketing
● Public Relations
● Business Administration
● Human Services
● Social Work
Equivalent relevant experience may be considered.
Schedule
● Part-time
● 25–30 hours per week
● Some evening or community event attendance may be required
Compensation
Competitive hourly pay based on experience.
