Description

Responsible for helping individuals, families, and groups of people to cope with problems they?re facing to improve their patients? lives

Collaborate with other professionals to evaluate patients? medical or physical condition and to assess patient needs.
Refer patient, or family to community resources to assist in recovery from mental or physical illness (if any).
Counsel patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
Plan discharge from care facility to home or other care facility.
Monitor, evaluate, and record of patient?s progress according to measurable goals described in treatment and care plan.
Identify environmental impediments to patient progress through interviews and review of patient records.
Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the patient.
Ensure the implementation of policies & procedures in order to achieve the hospital objective to enhance the care level & the patient experience.
Implement the departmental policies and procedures, local and international accreditation standards or requirements related to the department.
Actively involves in indicator development, data collection for the department and to measure the performance of the function, process, and outcome. Whenever required, conducts analysis and interpretation of data/information collected.
Ensures confidentiality of medical records and information.
Participate in hospital Safety Management & Risk Management Programs. Actively reports incidents in the system and whenever needed, participates in incident review, and root cause analysis.
Performs any other tasks within his/her competence as directed by the Customer Service Manager

Requirements

0- 2 years of experience in the related field
Bachelor Degree of Social Work or any related field.
