Role Summary:
The Dietician at GluCare Integrated Diabetes Center plays a key role in providing evidence-based nutritional care to patients with diabetes, obesity, metabolic syndrome, and other lifestyle-related conditions. The role emphasizes a patient-centered approach, integrating clinical nutrition therapy, technology-driven monitoring, and collaboration within a multidisciplinary team to achieve optimal metabolic outcomes and long-term health improvement.
Key Responsibilities
1. Patient Assessment & Nutritional Care:

Conduct comprehensive nutritional assessments, including dietary history, anthropometric measurements, and analysis of clinical and biochemical data.
- Develop individualized nutrition care plans aligned with patients’ medical conditions, treatment goals, and lifestyle.
- Provide medical nutrition therapy (MNT) for patients with diabetes, obesity, dyslipidemia, hypertension, and other metabolic disorders.
- Monitor and evaluate patient progress, adjusting care plans as needed.
- Educate patients and families on balanced nutrition, portion control, and sustainable dietary habits.
2. Multidisciplinary Collaboration:

Work closely with endocrinologists, diabetologists, physicians, diabetes educators, and health coaches to ensure integrated patient care.
- Participate in multidisciplinary case discussions and care plan development.
- Collaborate with the digital health team to interpret and act upon continuous glucose monitoring (CGM) and wearable data.
3. Technology-Enabled Nutrition Care:

Utilize GluCare’s digital health platform and EMR to track and analyze patient progress and engagement.
- Integrate data from wearable devices, mobile apps, and patient-reported outcomes into nutritional counseling.
- Support patients in adopting tech-enabled lifestyle management tools.
4. Education & Prevention:

Conduct one-on-one and group nutrition education sessions.
- Develop and contribute to patient education materials, workshops, and health awareness programs.
- Promote preventive nutrition strategies to reduce the risk of diabetes and metabolic complications.
5. Quality Assurance & Professional Development:

Maintain accurate and complete documentation in line with DHA and GluCare standards.
- Participate in clinical audits, continuous quality improvement, and professional education.
- Stay updated on current research, nutrition guidelines, and innovations in dietetics and metabolic care.
Understand, follow and support all hospital infection control programs
Requirements
Qualifications & Experience

Bachelor’s Degree in Nutrition, Dietetics, or equivalent from a recognized institution.
- Valid DHA License or Eligibility as a Dietician.
- Minimum 2 years of clinical experience, preferably in a diabetes, metabolic, or lifestyle-focused setting.
- Experience in digital health or technology-assisted care preferred.
- Strong knowledge of nutrition in chronic disease management (diabetes, cardiovascular disease, obesity).
- Proficiency in EMR systems and data interpretation.
