Key Responsibilities

1. Nutrition Assessment & Planning

  • Conduct detailed nutritional assessments for senior citizens, focusing on age-related needs and medical conditions.
  • Develop personalized diet charts for elders with chronic conditions such as diabetes, hypertension, cardiac issues, arthritis, renal concerns, and gastrointestinal disorders.
  • Modify diets based on appetite changes, mobility limitations, digestive issues, and medication schedules.

2. Ongoing Monitoring & Review

  • Regularly track progress and adjust meal plans to ensure optimal nutritional balance.
  • Monitor weight changes, hydration levels, blood sugar patterns, digestion, and energy levels.
  • Provide periodic reviews for caregivers and family members.

3. Collaboration & Communication

  • Work closely with doctors, nurses, physiotherapists, and caregivers to provide a holistic elder-care plan.
  • Guide Emopract’s field team and family members on food preparation, portion size, and safe handling of meals.

4. Elder-Specific Dietary Guidance

  • Plan diets supporting common elder needs: immunity, bone health, heart health, digestive ease, muscle strength, and cognitive wellbeing.
  • Address special issues such as poor appetite, chewing or swallowing difficulties, nutrient deficiencies, and dehydration risk.

5. Documentation & Reporting

  • Maintain accurate records of assessments, plans, and progress updates.
  • Provide monthly or weekly reports as required for family consultations or medical reviews.

6. Education & Counselling

  • Conduct short counselling sessions for elders and their families regarding healthy living, safe foods, meal timing, and lifestyle practices.
  • Provide guidance for diet during clinical symptoms (fever, infections, post-surgery recovery, etc.).

Skills & Qualifications

  • Bachelor’s or Master’s degree in Dietetics / Nutrition / Food Science.
  • Minimum 2 years of experience working with elder patients or in a clinical/healthcare setting.
  • Strong understanding of age-related nutrition needs and medical diets.
  • Excellent communication and counselling skills; ability to interact patiently with elders.
  • Ability to prepare easy-to-follow diet plans for caregivers.
  • Good documentation, follow-up, and tele-consultation skills.
