About This Role
SCAN Group and myPlace Health are seeking a strategic facilities leader to take full ownership of the physical infrastructure that supports our participants and care teams. In this role, you’ll be more than a project manager—you’ll be a steward of the SCAN and myPlace Health brand experience, shaping the physical environments where care happens. As a trusted partner to leadership and a mentor to a growing facilities team, you’ll guide everything from real estate strategy and long-term capital planning to complex construction projects and day-to-day operations, ensuring our centers are safe, welcoming, compliant, and always audit-ready.
We’re seeking a visionary builder and hands-on operator—someone who thrives on balancing high-level strategy with practical execution. You’ll strengthen vendor and landlord partnerships, build scalable processes, and shape a facilities program that has a direct, positive impact on both our participants’ daily lives and our care teams’ ability to deliver exceptional care.

What Does Success Look Like

Inspire and guide facility teams – Lead and mentor managers, supervisors, and technicians across multiple PACE clinical care sites, creating a culture of collaboration, proactive problem-solving, and accountability.

Be the trusted partner for leadership – Serve as the primary facilities liaison to the GLA & SLA leadership teams, offering strategic counsel and clear, consistent updates on all real estate and operational matters.

Shape the future through smart planning – Manage multi-site operational budgets and design a forward-looking 5-Year Capital Expense (CapEx) Plan to extend the life of building assets and support long-term financial health.

Champion safety and resilience – Partner with Compliance and Operations to build and own the facilities component of the company’s Business Continuity and Disaster Recovery (BC/DR) program while ensuring a safe and secure environment.

Keep operations running smoothly – Oversee day-to-day facility operations, working with landlords, establishing management processes, coordinating repairs, and managing vendor contracts to keep every site in top shape.

Set the standard for excellence – Develop and enforce SOPs for facility operations, from preventative maintenance to project intake, while ensuring maintenance and repair needs are prioritized and tracked.

Stay audit-ready and compliant – Ensure all facilities consistently meet standards and safety codes (DHCS, CMS, OSHA, ADA, Fire Marshal), protecting participants, staff, and visitors alike.

Drive strong vendor partnerships – Manage vendor contracts and relationships with a focus on both quality and cost-effectiveness.

Balance strategy with hands-on support – Provide high-level oversight while rolling up your sleeves when needed to guarantee seamless execution.

What Does Success Look LIke?

Bring a wealth of experience – 8+ years of progressive facility operations and management expertise, ideally in healthcare settings such as outpatient clinics, surgery centers, or adult day health centers.

Show financial leadership – Direct experience developing and managing both operational budgets and multi-year capital (CapEx) budgets.

Master the art of project delivery – Expert-level project management skills with a track record of identifying critical paths, anticipating risks, and avoiding cost overruns.

Ground your expertise in knowledge – Bachelor’s degree or equivalent professional experience.

Lead with heart and purpose – A true passion for serving high-risk seniors and frail older adults, helping them remain in the communities they love instead of institutional care.

Communicate and connect with ease – Exceptional interpersonal and communication skills, with proven success influencing, building consensus, and managing relationships across all levels of an organization.

Stay detail-focused while juggling priorities – Ability to manage multiple projects simultaneously while ensuring smooth execution of facility renovations.

Be a compliance champion – Deep knowledge of healthcare facility codes and safety standards, including ADA, OSHPD 3, DHCS, CMS, and Fire-Life Safety.

Navigate technical details with confidence – Skilled in reading and interpreting construction documents, drawings, and CAD files.

Leverage modern tools – Proficiency with project management and scheduling software such as Microsoft Project, Smartsheet, or similar platforms.

Stay connected to our community – Residency in the Greater Los Angeles area, with the flexibility to regularly travel to assigned facilities.
