Full Time Case Manager at Jail Transition Program

Join our team at Wasatch Behavioral Health as a Full-Time Case Manager with our On-Unit Drug Treatment Program and make a meaningful difference in the lives of individuals navigating mental health and substance use challenges. This onsite role is based at our Jail Transition Program in Spanish Fork, Utah, where you'll work directly with clients and their families in a supportive, team-centered environment. With a competitive wage of $21.15–$29.58/hour, you'll thrive in a workplace culture that values empathy, innovation, and collaboration while helping create a safe and positive atmosphere for those in need.

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Compensation: $21.15–$29.58 DOE and Licensure

What You'll Do as a Case Manager

  • Conducting client assessments and gathering background information
  • Developing individualized care plans tailored to client needs
  • Coordinating services with internal teams and outside providers
  • Monitoring client progress and adjusting plans as necessary
  • Collaborating with a multidisciplinary treatment team
  • Documenting sessions, updates, and service coordination
  • Helping clients identify goals and overcome barriers
  • Providing crisis support and problem-solving when needed
  • Offering empathetic, client-centered guidance throughout treatment

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.

  • Bachelor's degree with Social Service Worker (SSW) license
  • Bachelor's degree in any field with willingness to pursue SSW licensure

Additional Requirements:

  • Valid driver's license
  • Basic understanding of Social Work principles
  • Familiarity with social welfare policy and human development
  • Ability to problem-solve and maintain a client-centered approach
  • Must pass the Utah County Jail's criminal background check.
  • Spanish speakers may qualify for increased wages after testing
