Pay Rate: $17/hour

Job Summary:

Townhouse Center for Rehabilitation & Nursing is seeking a compassionate and reliable Home Health Aide (HHA) to join our team on a PRN basis. The ideal candidate will assist residents with transportation to medical appointments, provide support during outings, and ensure safety and comfort at all times.

Key Responsibilities:

• Escort residents to and from clinic appointments and dialysis sessions.
• Monitor and prevent falls or altercations, intervening promptly when needed.
• Assist residents in preparing for appointments and ensure snacks are provided during out-on-pass periods.
• Report all incidents and accidents promptly to the Clinical Coordinator or ADNS.
• Represent the facility professionally, adhering to all policies and procedures both on and off the property.
  
  
  

Minimum Qualifications:

• Current New York State Home Health Aide (HHA) certification.
• Good verbal communication skills and a flexible, dependable attitude.
