Join our dynamic home health care team as an Office Case Manager, where you will play a vital role in coordinating and managing patient care from our office. This energetic position offers the opportunity to work closely with healthcare professionals, patients, and families to ensure seamless delivery of home health services. You will oversee case management activities, facilitate communication among care teams, and support patients through their recovery and ongoing health needs. Responsibilities

  • Conduct comprehensive patient assessments to determine care needs, utilizing your knowledge of physiology, medical terminology, and clinical protocols.
  • Develop personalized care plans in collaboration with physicians, therapists, and other healthcare providers, ensuring alignment with patient goals and medical requirements.
  • Coordinate post-acute and outpatient services, including discharge planning and outpatient clinic follow-ups, to promote continuity of care.
  • Manage intake processes for new patients, verifying insurance coverage while adhering to HIPAA regulations to protect patient privacy.
  • Monitor patient progress through regular communication and documentation, updating medical records accurately and timely.
  • Facilitate utilization management by evaluating diagnostic evaluations, hospital records, and inpatient/outpatient data to optimize resource use.
  • Collaborate with hospital teams during transitions of care from ICU, ER, or surgical units such as OR or PICU to ensure smooth handoffs and appropriate follow-up.

Skills

  • Critical care experience in settings such as ICU, emergency room, or Level I Trauma Center environments.
  • Strong background in hospital medicine, infectious disease care, behavioral health, geriatrics, or medical-surgical units.
  • Knowledge of acute care procedures including vital signs assessment, diagnostic evaluation, and inpatient/outpatient management.
  • Experience with discharge planning, case management processes, and utilization management strategies.
  • Familiarity with medical records systems and medical terminology; ability to interpret complex clinical data.
  • Excellent patient service skills with a compassionate approach to patient care across diverse populations including pediatrics and post-acute patients.
  • Understanding of HIPAA compliance standards and the importance of maintaining confidentiality at all times.
