LPN (0-3) - Shift 1

Job Purpose:

The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER is responsible for providing high-quality nursing care to patients within the behavioral health setting. This role involves working under the supervision of registered nurses and physicians to ensure the delivery of compassionate and effective patient care. The LPN will play a critical role in supporting the healthcare team to promote patient well-being and recovery.

Key Responsibilities:

Administer medications and treatments as prescribed by healthcare providers.
Monitor and document patient vital signs and report any changes to the healthcare team.
Assist in the development and implementation of patient care plans.
Provide direct patient care, including wound care, personal hygiene, and assistance with daily activities.
Collaborate with the healthcare team to ensure a safe and therapeutic environment for patients.
Educate patients and their families about health conditions and care plans.
Maintain accurate and timely documentation of patient care and progress.
Participate in team meetings and contribute to the continuous improvement of patient care services.
Adhere to all policies, procedures, and regulatory requirements to ensure compliance and safety.

Required Education:

Completion of an accredited Licensed Practical Nurse (LPN) program.
Current LPN license in the state of employment.

Required Experience:

0-3 years of experience working as a Licensed Practical Nurse, preferably in a behavioral health setting.
Experience in providing direct patient care, including administering medications and monitoring vital signs.

Required Skills and Abilities:

Strong understanding of nursing care procedures and protocols.
Ability to work effectively in a team environment and collaborate with healthcare professionals.
Excellent communication skills, both verbal and written, for interacting with patients, families, and colleagues.
Proficient in using healthcare software and electronic medical records.
Ability to handle stressful situations calmly and effectively.
Strong organizational skills and attention to detail.
Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare.
