Job Summary
The Child & Adolescent Caseworker 1 – Outreach Program will receive training and instruction to ensure access to community agencies, services and people whose functions are to provide the support, training and assistance required for a stable, safe and healthy community life and to reduce the need for more formal agency intervention. Services are offered within the parameters imposed by funding and other resources.
DISTINGUISHING FEATURES OF THE CLASS:
This is a professional social services work in the Bradford County Human Service Agency. The focus of the position is to ensure continuity of care for all people who require mental health treatment, intensive services, and/or placement in specialized placements.
JOB FUNCTIONS:
1. Client Eligibility: The Outreach Program Case Manager (OCM) will ensure the client’s eligibility for entry into the program.
2. Responsibility for Continued Care: The OCM will monitor and assess the client’s continued need for the receipt of the services of the program.
3. Relationship to Other Parts of the System: The OCM will work closely with all behavioral health and ancillary service providers and will provide consultation in accordance with applicable confidentiality regulations.
4. Relationship to Other Parts of the System: The OCM will provide prevention and intervention activities in the schools and community, including drug and alcohol education and training to school staff and students; participation in community groups and coalitions aimed at prevention of alcohol, tobacco, and other drug use/abuse among students and community members; dissemination of educational materials to enhance awareness of drug and alcohol related issues.
5. Relationship to Other Parts of the System: The OCM will serve as Student Assistance Program Mental Health Liaison in assigned school districts and conduct groups for identified, at risk students.
6. Maintain appropriate documentation of contacts with children and their families, service providers, and schools’ districts. Document the service provided, outcome of the service and determine if follow-up is needed along with any state reporting requirements and data input into the state or joinder computer systems.
7. Participate in individual and treatment team discussions to prioritize referrals. Work with the child, family, school and providers to determine identified concerns and provide intervention and referrals to appropriate community services.
8. Maintain and follow the mandated client confidentiality policies including but not limited to HIPPA, 42 CFR Part 2, and 4 PA Code 255.5.
9. The OCM will assure that appropriate referrals are made to assist the child or adolescent in accessing more formal services including, but not limited to Mental Health Blended Case Management and/or Drug & Alcohol Case Management, when necessary.
10. Perform other duties as related to complete program objectives
PHYSICAL AND MENTAL REQUIREMENTS:
Must have the ability to record, convey, and present information, explain procedures, and follow instructions;
Must be able to sit for long periods of time with frequent periods of standing, walking, and occasional twisting, reaching, pulling, and pushing required for obtaining files, doing inspections or other job duties;
Dexterity requirements range from simple to complex movements of fingers/hands, legs/feet, or torso to carry out job duties;
Sedentary work with occasional lifting, 15-20 pounds;
Must be able to pay close attention to detail and concentrate on work.
Travels as necessary
REQUIRED KNOWLEDGE, SKILLS and ABILITIES:
Good knowledge of modern social services practices, casework philosophy and investigations, ability to deal with children and adults, Good communication and interviewing skills Must possess current Child Line, FBI Criminal History and Pennsylvania State Criminal History Clearances. Must possess a valid driver’s license. Must be able to work flexible hours.
MINIMUM EXPERIENCE and TRAINING:
Bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
or
Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
or
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions
ADDITIONAL TRAINING AND EXPERIENCE:
Must possess current Child Line, FBI Criminal History and Pennsylvania State Criminal History Clearances. Must possess a valid driver’s license. Must be able to work flexible hours.
