Nurse Practitioner/Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team across multiple locations.

• Provide comprehensive patient assessments, diagnoses, and develop care plans
• Order, interpret, and follow up on diagnostic tests
• Document patient care accurately in medical records
• Deliver care aligned with standard practices and collaborate with team members
• Manage patient progress and refer to specialists when necessary

Requirements include current state licensure, national certification, DEA license or eligibility, and BLS certification. The role is in-person, based in Sutherlin, OR, with relocation required before starting. Salary ranges from $125,000 to $150,000 annually.
