Practice Care Coordinator

Princeton Sports and Family Medicine, P.C., (PSFM) a growing and busy Wellness Center, that includes Sports Medicine, Family Practice & Physical Therapy located in Lawrenceville N.J. (close to Princeton) is a seeking a qualified professional to join our patient-oriented team.

PSFM is a well-established clinic that provides exceptional care to patients. PSFM focuses on diagnosing and treating patients in a kind, compassionate, care-centered, outpatient environment in a private clinic setting.

*** Great starting position for a new college grad or with 1-2 years of experience.

Immediate Need Full Time / Non-Exempt approximately 35-40 hours/week

Job Summary:

  • Works directly for the Practice Care Director and Practice Care Specialist as part of a coordinated team to improve patient care and support practice management, as needed.

Job Tasks and Responsibilities:

  • Provide administrative support to Practice Care Director and Practice Care Specialist in coordinating the day-to-day operations of the practice
  • Maintain providers’ schedule and coordinate provider follow-up appointments
  • Execute patient outreach calls to ensure patient preventative services are achieved, i.e Annual Physicals, Medicare Wellness Visits, Visit Programs, etc.
  • Collaborate with partner payers to achieve mutually defined quality, health, and costs of care goals with Value -Base Programs (Horizon, AmeriHealth, MIPS, etc.)
  • Assist in developing patient education materials
  • Maintain accurate records related to clinical performance metrics
  • Maintain strictest confidentiality while adhering to all HIPAA guidelines/regulations

Skills / Experience:

  • Knowledge of medical terminology would be a huge asset.
  • Knowledge of eClinicalWorks or other EMR system is a plus.
  • Must be able to work effectively and efficiently as part of a team.
  • Must be well organized, detail oriented and able to communicate effectively.
  • Must have a strong work ethic and willing to learn new things
  • Must have excellent problem-solving and critical thinking skills.
  • Should be very comfortable with technology.

Education:

College Degree – Health Informatics / Health Care Management/ Public Health

Additional Health Sciences, Technology, Computer Sciences, other Technology Management Courses or Training, a plus.
