Job Title: Patient Enrollment Specialist

We are seeking a talented Patient Enrollment Specialist to become a part of our vibrant team. This role requires a strong understanding of care management services, excellent communication skills, and the ability to collaborate with internal teams to meet clinical needs effectively.
Job Summary:
Nexus Healthcare Management is seeking a detail-oriented and compassionate Patient Enrollment Specialist to enhance our Care Management team. This role is pivotal in providing clinical and administrative support to care managers, ensuring efficient coordination of patient care, compliance with healthcare regulations, and seamless communication between providers, patients, and payers. The ideal candidate will have a clinical background, strong organizational skills, and a passion for improving patient outcomes.
Key Responsibilities:
· Conduct outbound calls to prospective patients for enrolment in care coordination programs.
· Clearly explain program benefits, eligibility, and enrolment steps in a compassionate and professional manner.
· Answer patient questions and resolve concerns to ensure confidence in joining the program.
· Use a consultative approach to guide conversations and improve patient engagement.
· Document call outcomes and maintain accurate records in the EMR or CRM system.
· Collaborate with clinical and administrative teams to ensure smooth handoff post enrolment.
· Follow up with patients who have shown interest in enrolment in Programs.
· Collaboration with the care management team for administrative tasks.
Experience:
A minimum of 02 Years of telehealth experience in a UK or US-based Healthcare Company is required.
Education:
MBBS, Physiotherapy, Pharmacy, Clinical Nutrition (clinical degree with prior sales experience is a plus)
