Job Summary

Tekboox, a US-based IT company, is urgently looking to hire Patient Care Coordinator (CCM) to support US-based Chronic Care Management programs.

Key Responsibilities

• Conduct monthly CCM calls with patients (minimum 20 minutes per patient as per CMS guidelines)
• Educate patients about their chronic conditions, medications, and care plans
• Document patient interactions accurately in EHR/EMR systems
• Coordinate care between patients, primary care providers, specialists, and care teams
• Monitor patient symptoms, adherence, and progress toward care goals
• Identify and escalate clinical concerns to supervising physicians or care managers
• Assist with medication reconciliation and preventive care reminders
• Support care plan updates under provider supervision
• Ensure compliance with HIPAA and CMS CCM documentation requirements

Required Qualifications

• MBBS degree (preferred) OR Bachelor other Allied Health Sciences
• Experience in patient care coordination, telehealth, or clinical support (preferred)
• Strong clinical communication and patient counseling skills
• Proficiency with EHR/EMR systems

Preferred Skills

• Prior experience with US CCM programs
• Knowledge of CMS guidelines for CCM
• Experience in remote/virtual healthcare settings
• Excellent English communication skills
• Ability to build long-term patient relationships

Job Location & Timing:

• Location: DHA Phase 3, Y Block, Lahore
• Shift: Night Shift (7:00 PM to 4:00 AM)
• Work Mode: Onsite, 5 days a week
