Pre-Visit Planner

JOB SUMMARY: Gather and arrange pertinent patient information ahead of their upcoming visit to enhance the visit experience and address any gaps in care.

JOB DUTIES:

• Conduct pre-visit planning chart reviews and coordinate with practices and patients to address and resolve quality measures and care gaps.
• Contact other providers, radiologists, labs, and immunization sources to collect records and results as necessary.
• Contact patients to gather necessary information.
• Use the EMR to communicate with practice administrators and other team members.
• Upload a completed pre-visit plan into the patient's EMR chart for the care team to review.
• Respond to all emails and answer calls to the concierge line.
• Attend meetings as required.
• Manage assigned projects and communicate project status to team, project leaders and management.

REQUIRED QUALIFICATIONS:

• Strong computer skills, including proficiency in Microsoft Office (specifically Word and Excel); knowledge of Teams a plus.
• Self-motivated, energetic, passionate, and innovative.
• A collaborative team player who follows systems and protocols to achieve shared goals.
• Highly organized with excellent verbal and written communication skills.
• Demonstrates sound judgment, decision-making and problem-solving abilities.
• Capable of maintaining confidentiality within compliance with practice, State and Federal regulations.
• Confident in communicating and reaching out to other community health care organizations and personnel.

PHYSICAL DEMANDS:

Pre-Visit Planner must be able to sit for extended periods of time and possess the manual dexterity required to operate computer systems and keyboard. Lifting 10 -15 pounds may be expected on any given day.
