Quality Assurance Physical Therapist
Job Summary
The Quality Assurance Physical Therapist is responsible for overseeing and improving the quality, compliance, and clinical performance of physical therapy services provided within the home healthcare agency. This role ensures that patient care meets regulatory standards, agency policies, payer requirements, and evidence-based clinical practices. The Quality Assurance Physical Therapist conducts chart audits, monitors outcomes, supports staff education, and collaborates with clinicians and leadership to maintain high standards of patient care and operational excellence.
Key Responsibilities
- Review and audit home health physical therapy documentation for accuracy, completeness, timeliness, and regulatory compliance.
- Ensure compliance with Medicare, Medicaid, state regulations, accreditation standards, and agency policies.
- Evaluate therapy care plans, OASIS documentation, progress notes, reassessments, and discharge summaries.
- Monitor patient outcomes, hospitalization rates, functional improvement measures, and quality indicators.
- Identify trends, deficiencies, and opportunities for clinical and operational improvement.
- Provide education, coaching, and feedback to physical therapists and therapy assistants regarding documentation standards and best practices.
- Assist clinicians with regulatory updates, payer guidelines, and quality improvement initiatives.
- Collaborate with nursing, occupational therapy, speech therapy, and administrative teams to improve coordination of care.
- Participate in case conferences, utilization reviews, and performance improvement meetings.
- Investigate patient complaints, incident reports, and compliance concerns related to therapy services.
- Support preparation for surveys, audits, and accreditation reviews.
- Develop and update therapy-related policies, procedures, and quality assurance protocols.
- Assist with tracking therapist productivity, visit utilization, and billing compliance.
- Maintain confidentiality of patient records and protected health information (PHI).
Qualifications
- Doctor of Physical Therapy (DPT) or Bachelor’s/Master’s degree in Physical Therapy from an accredited program.
- Current and unrestricted Physical Therapist license.
- Minimum of 3 years of clinical physical therapy experience, preferably in home health care.
- Previous quality assurance, compliance, or chart review experience preferred.
- Strong knowledge of home health regulations, OASIS documentation, Medicare guidelines, and Conditions of Participation (CoPs).
- Experience with electronic medical record (EMR) systems used in home healthcare.
- Excellent analytical, organizational, communication, and problem-solving skills.
Preferred Skills
- Knowledge of CMS home health quality measures and value-based purchasing programs.
- Ability to interpret clinical and operational data.
- Strong attention to detail and accuracy.
- Leadership, mentoring, and staff education experience.
- Ability to work independently and manage multiple priorities.
