We are expanding our referral network across Will County and are hiring an experienced healthcare relationship professional to develop and manage this territory.

This is a territory ownership role focused on long-term referral partnerships — not transactional sales.

Mokena

The selected candidate will own and develop this territory.

Position Overview

The Referral Partner Development Manager is responsible for building and maintaining strategic referral relationships with:

• Hospitals and discharge planners
• Case managers
• Rehabilitation centers
• Assisted living communities
• Skilled nursing facilities
• Specialty physician practices
• Memory care facilities
• Community healthcare organizations

The goal is to establish sustainable referral partnerships that generate qualified private-pay client introductions.

This role requires professionalism, credibility in clinical environments, and long-term relationship cultivation.

Core Responsibilities

• Identify and prioritize high-value medical referral sources
• Secure in-person meetings with decision makers
• Present Serenity’s private-pay care programs clearly and professionally
• Develop long-term referral partnerships
• Maintain consistent follow-up and engagement
• Track referral activity and territory pipeline
• Grow recurring referral volume within the territory

Required Qualifications

• Minimum 3 years of experience in healthcare sales, medical device sales, pharmaceutical sales, hospice sales, home health sales, or hospital liaison roles
• Experience working directly with discharge planners or case managers
• Demonstrated history of generating referral-based revenue
• Strong professional presence in medical environments
• Ability to work independently and manage a defined geographic territory
• Reliable transportation for field visits

Preferred Qualifications

• Existing relationships within Will County medical facilities
• Experience navigating hospital discharge systems
• Private-pay home care sales experience
• Experience managing suburban medical territories
• Familiarity with CRM systems
• Skilled nursing or assisted living network relationships

Compensation Structure

This is a performance-based role with uncapped earning potential.

• 7% commission on first-month collected revenue from new referred clients
• 10% commission when a referral partner generates three or more clients within a single month
• Commission applies across all Serenity service lines
• Commission is paid on collected revenue
• Commission continues for the life of the referral partner relationship while actively employed

• Establish 20+ facility introductions
