Duties

Assess patients and coordinate care, including discharge planning.
Organize and participate in family meetings.
Participate in the development of treatment plans.
Handle cases involving HMOs, IPAs, adoption/foster care, drug/alcohol abuse treatment options, homelessness, mental health crises, advance directives, POLST, and protective services.
Arrange DME, SNF, and ambulance transport independently.
Utilize computerized documentation systems effectively.
