Duties

  • Assess patients and coordinate care, including discharge planning.
  • Organize and participate in family meetings.
  • Participate in the development of treatment plans.
  • Handle cases involving HMOs, IPAs, adoption/foster care, drug/alcohol abuse treatment options, homelessness, mental health crises, advance directives, POLST, and protective services.
  • Arrange DME, SNF, and ambulance transport independently.
  • Utilize computerized documentation systems effectively.
