Registered Nurse (Fully Remote)

Remote Nurse Care Guide (Contract, 30–40 hrs/week)

At EZ Health, we help underserved seniors navigate the healthcare system so they can stay healthier, more independent, and out of the hospital.

We’re looking for nurses who want meaningful remote work that uses their clinical judgment, communication skills, and problem-solving ability in a high-touch, relationship-driven role.

This is a good fit for a nurse who loves helping patients, is great on the phone, and wants to make a real difference without the pace and stress of traditional bedside care.

Why this role is different

Small client load

You’ll work with a limited panel of about 20 clients, so you can give thoughtful, personalized support rather than being spread too thin.

Phone-first care

Most of your work will happen by phone. This is not a chat or messaging-heavy role.

Human-led, AI-supported

We use AI tools to help you work more efficiently, but the heart of the work is still human connection, judgment, and trust.

High trust, high impact

You’ll often be the person a senior turns to when they feel confused, stuck, or overwhelmed by the healthcare system.

What you’ll do

Build trusting relationships with seniors through compassionate, phone-based support
Help clients understand Medicare, benefits, and available resources in plain English
Coordinate care, including appointments, referrals, transportation, meals, and community resources
Solve problems creatively when the usual system is not enough
Work with doctors’ offices, health plans, insurers, and social services to help clients get what they need
Support seniors through real-life barriers that can affect their health and independence

Who this is for

A strong verbal communicator who connects well over the phone
Organized and able to manage multiple client needs at once
Comfortable with technology, including Google Docs, Google Sheets, and basic digital tools
A fast learner who enjoys solving real-world problems

We value different types of experience. Nurses with backgrounds in care coordination, case management, patient education, geriatrics, primary care, discharge planning, home health, community health, or telehealth may do especially well in this role.
