RN/LVN - Assisted & Independent Living

Responsibilities:

• Provide direct nursing care to residents in both assisted and independent living settings with empathy and professionalism.
• Conduct comprehensive assessments and develop individualized care plans tailored to each resident’s needs.
• Administer medications and treatments accurately in compliance with established protocols.
• Monitor and document residents’ health status and communicate any changes promptly to the healthcare team.
• Collaborate with interdisciplinary teams to ensure coordination of care and optimal outcomes.
• Educate residents and their families on health management and wellness practices.
• Maintain a safe and clean environment adhering to health and safety regulations.

• Current and valid RN or LVN license in the state of practice.
• Minimum of 1 year nursing experience, preferably in assisted or independent living or similar healthcare settings.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities with residents, families, and colleagues.
• Ability to work independently as well as collaboratively in a team environment.
• Compassionate, patient-centered approach focused on quality care.
• Basic computer skills for documentation and electronic health record use.
