<li>The Home Care Nurse assistant shall be accountable for the clinical, educational, quality of care provided to the patients, using as reference the policies, procedures, guidelines and professional practice of hospital.</li>

Main Duties and Responsibilities/Performance Standards:

<li>Teach and aware family members on all safety measures to be taken for the new born or the elderly patient.</li>

<li>Inform the supervisors and families in case of any changes or deterioration in the client&rsquo;s condition.</li>
<li>Bath the client (new born, bed ridden, ambulatory patient)</li>
<li>Dress the client (from assisting to total dependent dressing)</li>
<li>Assist the patient to eliminate his feces (assisting with bedpans and urinals, providing incontinent care as needed, assisting patient to bathroom)</li>
<li>Do the bedding for bed-ridden patients.</li>
<li>Catheter care (emptying drainage bags, recording intake and output)</li>
<li>Assist in feeding (feeding patient, serving meals, assisting with hydration needs, prepare formulas for new born)</li>
<li>Help with Range of motion exercises and position the patient when needed to prevent bedsores.</li>
<li>Assist in ambulation or transporting patient in wheelchair and ambulation with walkers and canes.</li>
<li>Organize patient&rsquo;s room, preserve his personal belonging.</li>
<li>Read for the patient if required.</li>
<li>Perform Post mortem care when needed.</li>
<li>Report any misuse or damage of any instrument or equipment used.</li></ul>
