 PRIMARY FUNCTION

 Responsibilities And Tasks

 Utilize an electronic medical record to maintain accurate injury, OSHA and workers compensation recordkeeping through timely documentation of all interactions, clinical care and case management activities.

 Serve as a member of the Safety and Ergonomics committee, supporting safety and ergonomic functions at the location, and provide leadership in recognizing and preventing workplace hazards

 Develop wellness initiatives and programs that align with company goals and direction

 Lead ergonomics initiatives, analyze injury data, identify trends and implement improvement efforts. Manage the Ergonomics Job Measurement System.

 The Triage Nurse is a remote Registered Nurse who provides telephone and electronic triage support to firsthand individuals and staff, while also supporting outpatient care coordination. This is primarily a day-shift role (8 hours/day, 8:30-4:30 PST or 8:30-4:30 PST), with occasional potential for nights or weekends. When not managing acute issues, Triage Nurses focus on care coordination, training, and related administrative tasks.
 
 
 Responsibilities of a Triage Nurse include:

 Triage and Escalation:
 
 Manage inbound clinical issues from firsthand staff and patients via phone; triage appropriately and escalate emergencies immediately.

 Collaboration:
 
 Work closely with peer mental health workers, social workers, and APPs to address acute issues comprehensively.
 </li>
 <li>
 
 Coordination:
 
 Coordinate care with patients' other providers to ensure seamless health management.
 </li>

 Training:
 
 Develop and deliver training on basic medical topics for peer mental health workers and social workers
 
 
 </li>
 </ul>
 Triage Nurses should have:
 
 
 <ul>
 <li>
 Strong triage and prioritization skills, with the ability to rapidly assess and determine the appropriate level of care.
 </li>
 <li>
 Problem-solving expertise with a creative, patient-centered approach.
 </li>
 <li>
 Ability to provide condition-specific patient education and self-management guidance.

 Required experience includes:

 Active RN license through a Nurse Licensure Compact (NLC) state and willingness to obtain licensure in non-compact states.

 Bachelor of Science in Nursing (BSN).
 </li>
 <li>
 At least 3 (three) years of clinical care experience in an Emergency Department.
 </li>
 <li>
 Experience working with populations facing challenges such as behavioral health and/or substance use disorders.
 </li>
 <li>
 Care management and coordination experience.
 </li>
 <li>
 Bilingual in Spanish

Bilingual Triage Nurse

MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency's clinics. This includes diagnosis, treatment, consultation and follow-up care. This individual attends to the patient's need with the utmost patient care, compassion and understanding. This work is carried out in support of the mission. DUTIES AND RESPONSIBILITIES: <ul><li>Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program.</li><li>Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.</li><li>Records accurate medical histories, physical findings and formulates plan and prognosis based on patient's condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation.</li><li>Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan.</li><li>Submits health care plan and goals of individual patients for periodic review and evaluation by physician.</li><li>Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures.</li><li>Refers patients to physician or to specialized health resources for treatment.</li><li>Makes patients aware of importance of a healthy lifestyle.</li><li>Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments.</li><li>Analyzes new data gained from conferences, training workshops, professional literature and on the job training, where they will assimilate it into their clinical work.</li><li>Keeps abreast of changes in field.</li><li>Performs other duties as requested.</li></ul> REQUIREMENTS: Education, Experience and Skills Required: <ul><li>Master's degree from an accredited advanced Nurse Practitioner program</li><li>At least Two (2) years' experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs</li><li>Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs</li><li>Ability to demonstrate sensitivity to patients with a variety of psycho social issues</li></ul> License/Certification Required: <ul><li>NYS RN and NP (Adult and Family specialty areas) licensures.</li><li>Valid DEA registration.</li><li>eMedNY Provider.</li><li>Valid NPI.</li><li>BLS Certification.</li></ul> Preferred Education, License/Certification, Experience and/or Skills: <ul><li>National Board Certification within 12 months of hire.</li><li>ACLS certification.</li><li>Familiarity with OASAS, OMH, and DOH regulations.</li></ul> We are an equal opportunity employer and comply with all applicable federal, state, and local fair employment practices laws.
