Social Service Case Manager

• 
  Initial Intake: Direct all entry procedures for new clients.
• Care Planning: Participate in the initial assessment process and the formal development of the service plan of care.
• Documentation & Compliance: Maintain accurate, up-to-date patient records, ensuring compliance with health regulations
• Resource Navigation: Connect patients with appropriate community services, social services, and support resources.

• Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.

• High School diploma equivalency with 2 years of cumulative experience OR Associate's
  degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
