Location: Arlington Heights, IL
Facility: Ascension Center for Mental Health
Department: Supervised Residential Program
Schedule: Day shift | Full-time | 8:00 am - 4:30 pm
Salary: $23.18 - $32.32 per hour
What You Will Do:
Initial Intake: Direct all entry procedures for new clients.-
Care Planning: Participate in the initial assessment process and the formal development of the service plan of care.

Documentation & Compliance: Maintain accurate, up-to-date patient records, ensuring compliance with health regulations

Resource Navigation: Connect patients with appropriate community services, social services, and support resources.
What You Will Need:
Licensure / Certification / Registration:
-
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
Education:
-
High School diploma equivalency with 2 years of cumulative experience OR Associate's
degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences:
Bachelor's Degree in Social Work, Psychology, or Counseling required
Supervisory experience is highly preferred
Must have a valid driver's license
This role provides oversight and supervision to clients, staff, and interns at our Residential Facility
