Position Summary

We are seeking a qualified and compassionate Social Services professional to join our healthcare team in Jacksonville, Florida. This role is responsible for providing comprehensive psychosocial support, care coordination, and resource management services to patients in accordance with Florida state regulations and federal healthcare guidelines. The ideal candidate will play a critical role in addressing social determinants of health, improving patient outcomes, and ensuring continuity of care.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, financial, and environmental needs.
  • Communicate effectively with patients and families to address concerns, provide guidance, and coordinate appropriate services.
  • Coordinate referrals to community agencies, behavioral health providers, housing resources, financial assistance programs, and support services within Jacksonville and surrounding areas.
  • Verify and update patient insurance information and assist patients in understanding coverage, benefits, and eligibility.
  • Maintain and update Primary Care Provider (PCP) and related patient data within internal systems.
  • Provide crisis intervention and short-term counseling as needed.
  • Ensure accurate and timely documentation within EMR systems in compliance with HIPAA, CMS, and Florida state regulations.
  • Advocate for patient rights and promote culturally competent and ethical care delivery.
  • Perform general administrative duties related to case management and care coordination.
  • Collaborate with internal departments to resolve patient-related concerns and remove barriers to care access.

Required Qualifications

  • Master’s Degree in Social Work (MSW) from an accredited institution.
  • Strong knowledge of Medicare, Medicaid, and commercial insurance processes.
  • Understanding of Florida healthcare compliance standards and federal regulations.
  • Excellent verbal and written communication skills.
  • Strong organizational skills with the ability to manage multiple cases in a fast-paced environment.

Preferred Qualifications

  • Experience working within the Jacksonville, FL healthcare network or community resource system.
  • Familiarity with EMR systems and value-based care models.
  • Bilingual skills (English/Spanish) preferred.
  • Experience supporting diverse and underserved populations.

Work Environment

This position operates within a clinical or healthcare office environment in Jacksonville, Florida. The role may require direct patient interaction and coordination with external community agencies. Compliance with federal (HIPAA), CMS, and Florida state healthcare regulations is mandatory.
