Social Worker III – Adult Services
SOCIAL SERVICES

GENERAL STATEMENT OF DUTIES
This position will primarily provide case management services to SA-In Home clients and Guardianship clients. This position may also be assigned responsibilities with Adult Protective Services and Outreach cases. Intensive social work services include comprehensive assessments and planning with mediating conflicts; counseling clients and families facing long-term care decisions, the psychosocial aspects of aging, and chronic and terminal illness; and crisis intervention. Comprehensive knowledge and brokering of home and community-based services is coupled with advanced social work techniques in the programmatic areas of long-term care, personal care services, and at-risk case management.

EXAMPLES OF DUTIES PERFORMED
  • Completes assessments to determine needs
  • Develops care plan to meet both long and short-term goals and advocates for the client’s right to self-determination to the extent that the client is capable of making competent decisions
  • Authorizes the provision of services and coordinates with community-based services
  • Provides ongoing supervision of the service plan to assure proper implementation and make appropriate changes as necessary
  • Provides individual and family counselling and supportive therapy
  • Rotates on-call responsibilities with Adult Services team to insure 24/hour coverage of APS reports
  • Coordinates the provision of services for DSS wards; preparing court status reports and maintaining appropriate communication with the DSS Director.


RECRUITMENT STANDARDS
Knowledge, Skills and Abilities
Basic computer skills, organizational skills, ability to prioritize and multi-task, effective communication skills. This is in addition to social work skills and experience including assessment and counseling, mediation, medical terminology and knowledge of adult services. A high level of skill in independent decision making is required.

MINIMUM EDUCATION AND EXPERIENCE
BSW from an accredited School of Social Work in an accredited college or university or 4-year degree in a human services field plus one year directly related experience, or 4-year College or university degree plus two years of directly related experience. Directly related experience is defined as, “human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.”
