Please note: The salary range above represents this position’s earning potential. The anticipated hiring range for this position will be $80,070 - $122,462 based on the candidate’s qualifications and experience.

WHO WE ARE LOOKING FOR

Aging and Disability Services is seeking a compassionate Licensed Clinical Social Worker (LCSW-C) to provide clinical social work services to vulnerable adults, 18 and over, who have disabilities and receive care and supervision in community based residential care milieu. This social work case manager role also will certify and monitor CARE family homes and complete yearly re-certification of assigned CARE homes as required by the State. Within-caseload allegations of abuse, neglect, self-neglect and exploitation might be assigned for investigation by the AFC social work case manager, who also is assigned to do monthly Adult Protective Services Back-Up (BUP) coverage, as well as unit coverage duty on an as needed basis.

What You'll Be Doing

Adult Foster Care (AFC) Program participants are unable to live independently in the community due to mental illness, intellectual, cognitive or physical disability. The case management responsibility includes:

  • A comprehensive ongoing assessment of the adult’s needs and resources;
  • Development of an individualized service plan designed to address the adult’s needs;
  • Referral to and coordination of appropriate resources;
  • Provision of counseling and other services directly to the adult;
  • Advocacy with and on behalf of the adult;
  • Ongoing monitoring and evaluation of the effectiveness and efficiency of services as well as adequacy of the plan for service continuation.
  • Facilitation of a higher level of care if no other less restrictive safe option is available
  • Documentation of all client and collateral interactions and case activities in the Child Juvenile and Adult Management System (CJAMS) electronic case management database.
  • Other duties as assigned.
The selected candidate will be required to successfully complete a pre-employment medical exam, drug screen, and a background check.

Minimum Qualifications

Education: Possession of a Master’s Degree in Social Work (MSW) from an accredited college or university that meets the criteria of graduate social work program accredited by the Council on Social Work Education.
Experience: Two (2) years, to include a minimum 3,000 hours of professional experience rendering social work services under the supervision of a LCSW-C in accordance with Maryland Board requirements.
License:
  • Must be licensed as a Licensed Certified Social Worker - Clinical – LCSW-C by the Maryland State Board of Social Work Examiners prior to appointment. Note: There will be no substitutions for this requirement.
  • Possession and maintenance at all times of a valid Class C (or equivalent) driver’s license from the applicant’s state of residence when required for job-related duties.

Preferred Criteria, Interview Preferences

The applications of those individuals meeting the minimum qualifications will be reviewed to determine the extent, relevancy of training, and experience in the following areas:

  • Casework experience serving vulnerable populations, including adults with disabilities & the frail elderly.
  • Experience with electronic case management databases.
  • Demonstrated experience with interdisciplinary teaming, community outreach, and customer service.
  • Experience in applying social work principles, theories, and methods relevant to the client population.

Preference for interviews may be given to candidates with demonstrated ability toethically and autonomously practice within the required scope of service.
