About the Role

Are you passionate about empowering people with disability to live their best lives?

As a Support Coordinator, you'll play an essential role in helping NDIS participants understand and implement their plans, build their capacity to manage their supports, and connect with their communities.

You'll work closely with participants, families, service providers, and the NDIA to ensure individuals receive the right support, at the right time, to achieve their goals.

This is a casual position with the opportunity to transition to part-time or full-time employment for the right candidate. The role offers flexibility and room to grow within a supportive, values-driven organisation.

Key Responsibilities

  • Assist participants to understand and implement their NDIS plans, build capacity, and increase independence.
  • Help participants connect with community, mainstream, and funded supports.
  • Strengthen informal support networks and promote community participation.
  • Support participants to develop service agreements, negotiate with providers, and manage budgets.
  • Coordinate services and resolve issues, acting as the key contact for service changes, complaints, or plan reviews.
  • Prepare participants for NDIS plan reviews, helping them evaluate progress and set new goals.
  • Liaise with the NDIA, plan managers, and other stakeholders to ensure smooth plan implementation and reporting.
  • Maintain confidentiality, manage potential conflicts of interest, and uphold Shoreline's Code of Conduct.

What You'll Bring

  • A genuine commitment to person-centred, strengths-based practice.
  • Excellent communication and active listening skills, with the ability to adapt to individual needs.
  • Strong problem-solving, negotiation, and conflict resolution skills.
  • High level of organisation, time management, and administrative ability.
  • Confidence using technology, NDIS systems, and participant portals.
  • Knowledge of the NDIS Price Guide, relevant legislation, and reasonable and necessary supports.
  • Ability to build strong relationships with participants, families, providers, and community networks.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.

Essential Requirements

  • A Working with Children Check (WWCC)
  • A National Police Check (required on application)
  • A valid NDIS Worker Screening Check (or willingness to obtain prior to commencement)
  • Relevant industry qualification (preferred, e.g. Community Services, Social Work, Disability, or similar)
  • Relevant industry experience within the disability, community, or human services sector
